Important Immunization Information

Colorado law requires students who attend a public, private, or parochial kindergarten - 12th grade school to be vaccinated against certain vaccine preventable diseases, unless a Certificate of Exemption is filed. For more information, visit cdphe.colorado.gov/schoolrequiredvaccines.

Students entering sixth grade must receive one dose of Tdap vaccine prior to entry, even if they are younger than 11-years-old. You can view recommended vaccine schedules at cdc.gov/vaccines/schedules/parents-adults/resources-parents.html

Your student may be excluded from school if we do not have an up-to-date Certificate of Immunization, Certificate of Exemption, or in-process plan on file for your student.

Adams 12 will be enforcing school exclusion dates, per Colorado Revised Statutes 25-4-902. Thornton Middle School exclusion date is Wednesday, October 26, 2022. 

Thornton Middle School health office has sent letters through USPS to all families whose students are not currently compliant with their school immunization requirements. For those students, you must provide one of the following to Thornton Middle School on or before October 26 in order to comply with Colorado law:

  1. An immunization record from your health care provider or a Certificate of Immunization certifying that the student has received the necessary immunizations as indicated above.

  2. If a student’s Certificate of Immunization is not up to date, the parent, guardian, or emancipated student has 14 days after notification to provide documentation that the next required immunization was administered and submit a written plan for completion of any additional required immunizations. If the plan is not completed, the student shall be excluded from school for noncompliance.

  3. A medical or non-medical exemption is submitted to the school if a student is not fully immunized. Exemption guidance and information are available at www.colorado.gov/vaccineexemption.
